Our Goal
Community Mental Health Services provides structured support, life skill development, and case management assistance to adults with a mental health diagnosis who demonstrate a need for greater stability and personal management within their home and community/ Services are provided following a determination of eligibility and the completion of an assessment to identify the individual’s needs. Services are provided by trained Mental Health Support Professionals who provide a variety of supports to include training and skill building,
• Functional skills
• Behavioral management
• Health and safety awareness
• Activities of daily living
• Assistance with medication management
• Monitoring of health, nutrition, and physical health
• Assistance with budgeting and money management
Our goal is to support individuals with valuable tools to increase their self sufficiency and personal management thereby reducing the likelihood of hospitalization, incarceration and any other condition that could stifle their ability to thrive at home or in the community.
Individuals who are:
• Adults 18+ years old
• Experience difficulty managing daily life skills and relationships with others
• Are prescribed medication to treat mental, behavioral or emotional challenges
• Have been hospitalized due to mental, behavioral, or emotional challenges
